Description

VA NORTHERN INDIANA HEALTHCARE SYSTEM Fort Wayne Campus PROJECT: 610A4-21-202 Add Card Readers STATEMENT OF WORK: Contractor will provide all labor, equipment, materials and supervision necessary to add HID card readers which will require the modification/upgrade of doors and hardware in Buildings 1, 2, 5, and 17 as listed in the specifications and drawings. The primary monitoring shall occur at the VA Police room 101 in Building 1 at the Fort Wayne Campus and the VA Police Dispatch at the Marion Campus. The system shall have a push button to secure/unsecure all exterior doors that can only be accessed by the VA police. The system shall be installed and fully tested in compliance with VA Handbook 0730/4, Appendix B, the VA Physical Security Resilience Design Manual, as well as the specifications and drawings for this project. This project will be conducted in phases to mitigate interruptions in patient care and facility accessibility in accordance with VA provided drawings and specifications. All equipment installed shall be compatible with and able to tie into the VA s current Lenel system. All equipment shall have the ability to be programmed and maintained by Stanley Security who provides programming and maintenance services for the VA s security system. Contractor must be in compliance with all VA security polices before access to security system will be granted. Construction shall adhere to the drawings and specifications. Prior to final payment to the contractor the Card Readers that have been installed must be able to be read by and communicate with the VA system. All work to be completed at the VA- Northern Indiana Health Care System Fort Wayne Campus, 2121 Lake Ave., Fort Wayne, IN 46805, VA-Northern Indiana Health Care System Marion Campus, 1700 E 38th St, Marion, IN 46953. Work to be completed within 240 calendar days from notice to proceed. JUSTIFICATION OF WORK: This project is designed to bring the Fort Wayne campus into compliance with VA Handbook 0730, and VA Physical Security Resilience Design Manual. RECORD RETENTION & STORAGE (1) Federal Records related to this contract, regardless of medium, shall be created, maintained, and dispositioned in accordance with the VHA Records Control Schedule (RCS) 10-1, General Records Schedule (GRS) or another NARA-approved records schedule, if applicable. (2) Federal Records shall be stored per the National Archives & Records Administration (NARA) Facility Standards for Records Storage, as described in 36 Code of Federal Regulations (CFR) 1234.
