This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

1) The Indiana Army National Guard is soliciting W912L9-23-Q-0003 as an RFQ

2) This Firm Fixed Price (FFP) procurement is a 100% Small Business set-aside in accordance with (IAW) the Federal Acquisition Regulation (FAR) Part 19.502-2 (b).

3) The CLINs listed below identify the Brand Name (or Equal) Specifications the requiring activity is looking for.

4)Please price any delivery/Installation/removal costs associated within the individual CLIN and not on a separate clin.

4a) The quote needs to include the costs associated with mounting equipment to the floor/wall as specified by the CLIN as well as the removal/disposal of old units. All new units will need to be hooked up and tested.

5) Contract will be awarded as firm-fixed price.

CLIN 0001 - Primo 20 Quart Mixer - Model PM-20

CLIN 0002 - Winholt Warming Cabinet - Model INHPL-1836C-DGT (Floor Mounted-Concrete) (Removal of Old Unit)

CLIN 0003 - True T72-HC Refrigerator - (Wall Mounted) (Removal of Old Unit)

CLIN 0004 - Black Diamond Gas Convection Oven - Model BDCOF-54/NG (Floor Mounted-Concrete) (Removal of Old Unit)

6. Quotes shall include the following:

Discount Terms

A technical description in sufficient detail to evaluate similarity with the specifications associated with the Name Brand/Model Number listed above.

Clauses 52.204-24 and 52.204-26 must be reviewed and checked accordingly.

7. The Government will award a contract resulting from this RFQ to the responsible offeror whose offer conforming to the RFQ will represent the BEST VALUE to the Government.
